There is growing interest in the treatment of smoking-induced chronic obstructive pulmonary disease (COPD), a major cause of mortality and morbidity in primary and secondary prevention. The most effective and safe method of treatment of smoking-induced chronic obstructive pulmonary disease (COPD) is the combination of a short-acting bupropion (bupropion, Zyban®) with a long-acting chlorpheniramine (CataGen®) to reduce cigarette smoking and cravings. A combination of bupropion and CataGen® to reduce the severity of cravings is particularly useful for patients with COPD. As a result, patients are more likely to be successful in reducing the severity of their symptoms.
Bupropion is an antidepressant medication that is approved for the treatment of major depressive disorder (MDD) and is approved for the treatment of nicotine dependence in patients with MDD. Bupropion has been found to reduce the risk of relapse of MDD by 40-80% in both the short-term treatment and long-term treatment settings. However, its use in patients with COPD has not been well defined. In addition, the use of bupropion is associated with a risk of adverse effects such as weight gain, sedation, and QTc prolongation.
To assess the efficacy of bupropion in patients with COPD and to evaluate its potential risks to the patient. Patients with COPD who received placebo, bupropion and/or catagen in the previous 12 weeks were enrolled. Treatment was conducted in the hospital, after 12 weeks of treatment. The primary outcomes were clinical symptoms of COPD (elevated dyspnea, respiratory distress, and/or cough) and weight-related adverse events. Secondary outcomes included hospitalizations, length of stay, and hospital length of stay. The primary efficacy endpoint was change in weight-related adverse events.
This was a retrospective cohort study. The primary study objective was to assess the effect of bupropion on clinical symptoms of COPD.
The study population consisted of patients with COPD who received placebo, bupropion and/or CataGen® in the previous 12 weeks.
The primary study population was patients (n = 45) who received placebo, bupropion and/or CataGen® in the previous 12 weeks. The primary efficacy endpoint was change in weight-related adverse events (n = 30).
Bupropion may be an effective treatment option for patients with COPD, but it may have a small risk of weight gain, sedation, and QTc prolongation. Therefore, it should be reserved for patients with severe COPD who require a lower dose of bupropion, or for patients with moderate to severe COPD who are not candidates for a lower dose of bupropion. Additionally, patients should be aware of the potential risks associated with bupropion. Patients should be informed about the potential risk of weight gain, QTc prolongation, and sedation.
